A Personalized Health Enhancement Plan

Summary and Recommendations

At the conclusion of your day-long, on-site One experience, you will receive a monogrammed, leather-bound binder that contains a detailed report of your results from the day’s testing and consultations. Dr. Peña will personally review all of the results with you, highlighting specific areas that may need special attention. Mutually agreed upon goals are defined for the upcoming year which are derived specifically from your individual results. Along with the report analysis, you will review these goals, and define strategies toward achieving them, with Dr. Peña. We pride ourselves in simplifying the medical jargon and taking as much time as is needed to help you comprehend all findings.

Following your visit with us, Dr. Peña will contact you within three weeks to again review your report highlights, answer any questions that may have come up in the interim, and alert you of any test results that may have arrived after your visit. At this point, 20 physician hours will have been dedicated to your One experience plus the hours that the staff and clinicians spend from preparation through the final report – a yet unprecedented amount of dedicated medical staff time within the category of “super-physicals.”

The Report

Our typical comprehensive report outlines your Health History, Family History, Lifestyle History and any history of significant Diagnostic Testing. A Review of Systems is also noted. The results of you physical exam are summarized as are all lab and diagnostic testing. A detailed print out of lab results is contained elsewhere within the binder – and upon your direction will be relayed to your primary care physician. The final section of the report – and perhaps the most significant to our clients – provides an assessment of all of the information gathered through the process and a plan to follow for the upcoming year in order to optimize long term health.
